What is a Health Coach?
A health coach is a registered nurse (RN) who helps you implement a personalized plan to improve your health based on your personal health goals and lifestyle. A health coach will hold you accountable to your personal goals and track your progress over time.
A health coach is not a replacement for your primary care physician. Instead, a health coach will work with you and your physician to determine the most important areas of your health to improve.
How can a Health Coach help me?
Patients with chronic conditions like diabetes, asthma, COPD and hypertension can work with a health coach to improve their health. Health coaches also assist patients who need help with medication education, weight management, nutrition, and answers to their health questions.
Health coaches can help patients of any age. In fact, our health coaches have experience working with parents to improve their children's health through identifying methods of exercise and managing nutrition.
If you think a health coach could help, one probably will!
