Care Navigation Clinic

The Physician Network manages a Care Navigation Program for patients being discharged from CHI Health St. Elizabeth or CHI Health Nebraska Heart who require ongoing care at a skilled nursing facility or rehabilitation center. The program reduces the chance of an individual needing to be readmitted to the hospital and promotes safe recovery.

Each patient participating in the Care Navigation Program will be assigned a Care Navigator, who helps to coordinate care and keeps the patient’s primary care provider updated on the patient’s status. A Care Navigator will typically visit a patient two to five times each week, depending on the patient’s condition. Care Navigators are also available on nights and weekends as needed, ensuring excellent care and responsiveness for all patients.

We work with several facilities to ensure a smooth transition, open communication, and continued high-quality care for all patients in the Care Navigation program. All partner facilities have undergone rigorous site reviews and meet or exceed Nebraska state regulations for federal quality standards and patient satisfaction ratings.

Care Management Staff
