Pain and Physical Medicine Appointments
We are located at 908 N Howard Avenue, Suite 105, in Grand Island, NE. To make an appointment, please call us at (308) 398-8900. We schedule appointments between 8 a.m.-12 p.m. and 1 p.m.-5 p.m.
New patients begin their treatment with a one-hour evaluation session. Follow-up appointments are typically 30-minutes long. Please bring all medications with you on your first visit, including all non-prescription medications or remedies you currently use.
Please have all necessary information available prior to calling for an appointment so that we can plan the best time for your visit. If you feel an appointment is needed on the day you call, please call as early in the day as possible.
If you are a new patient to the Pain and Physical Medicine Clinic, we ask that you arrive 20 - 30 minutes prior to your scheduled time to complete the necessary paperwork. You can also print these forms off in advance, complete them and bring them with you to your appointment. Forms are available on the “Patient Forms” page of this website.
We ask that you provide us with your health insurance card and driver’s license to copy for your medical record. The driver’s license is only used for patient identification.
