To make an appointment, please call between the hours of 8:00 a.m. - 4:30 p.m.

If you are a patient of Elizabether DiPerna, PA-C, please call (402) 327-7505

Please have all necessary information available before calling so we can schedule your appointment at an appropriate time. If you feel an appointment is needed on the day you call, please call as early in the day as possible.

Please bring your photo ID and current insurance card with you to each visit. This helps us verify your identity to protect your private health information. Also, please bring a list of all current medications you're taking, including any over-the-counter medications, vitamins and herbal supplements to your appointment.

New Patient Information

If you are a new patient to Stevens Creek Family Medicine, we ask that you arrive approximately 30 minutes prior to your scheduled time to complete the necessary paperwork. Upon arrival at our clinic, we will ask that you complete our Patient Information Sheet and Adult or Pediatric History Form. If possible, we ask that you print the Patient Information sheet off in advance from our website, complete it and bring it with you to your appointment.

If you are a Medicare patient and being seen for the Welcome to Medicare Physical, or the Medicare Annual Wellness Exam, we ask that you also print these forms off in advance from our website, complete them and bring them with you to your appointment. This will help with the check-in process.
