We are able to provide same-day appointments for acute illnesses like colds, flu, sinus infections, bronchitis and minor injuries.
For refills, please notify your pharmacist 7 days prior to running out of medication to prevent undue lapses in your medication schedule. All prescriptions and authorizations for refills must be requested during normal clinic hours. Not all prescriptions can be refilled over the telephone. If you have no refills remaining, you may need to schedule an appointment with your physician.
We are able to provide same-day appointments for acute illnesses like colds, flu, sinus infections, bronchitis and minor injuries. We encourage you to call us as early in the day as possible to ensure an appointment will be available the same day.
Here at Stevens Creek Family Medicine, our lab is open from 7:30 a.m. - 5:00 p.m., Monday through Friday. If we are unable to perform the test here, we will draw the specimen and send it to our Elizabeth Park South Lab. For most lab tests, your results are available between 48-72 hours.
It is very important to the providers here at Stevens Creek Family Medicine to provide preventive medicine to our patients and to encourage a healthy lifestyle. We encourage you to read the information listed below prior to your physical.
Complete Physical/Preventative Visit
If you are scheduled for a physical with one of our providers, we ask you to change into one of our patient gowns prior to the provider coming to see you. The gown will help the provider in doing a thorough evaluation (i.e. looking for abnormal moles, skin tags, listening to your heart and lungs, etc.)
This visit is strictly to discuss preventative measures that you and your provider feel appropriate for your age and medical condition. We want to keep you in the best possible health.
Problem Focus Issues and Concerns
If you want to talk about additional problems or concerns when you come to our clinic for a Complete Physical/Preventative Visit, then your physical might need to be rescheduled so we are able to discuss those medical concerns separately.
Medical insurance does not cover physicals and additional medical problems being discussed on the same day of service. If you have any questions or concerns about the above information, please check with the Nurse/Medical Assistant at your appointment. We suggest a second appointment when we know that it could save you additional cost.
The x-ray department in our clinic is available Monday - Friday, 8:00 a.m. - 5:00 p.m. We schedule mammograms, ultrasounds, MRIs and CT scans at the facility that is most convenient for you or according to your health insurance.

What is a Health Coach?
A health coach is a registered nurse (RN) who helps you implement a personalized plan to improve your health based on your personal health goals and lifestyle. A health coach will assist you in being accountable to your personal goals and track your progress over time.
A health coach is not a replacement for your primary care provider. Instead, a health coach will work with you and your provider to determine the most important areas of your health to improve.
How can a Health Coach help me?
Patients with chronic conditions like diabetes, asthma, COPD and hypertension can work with a health coach to improve their health. Health coaches also assist patients who need help with medication education, weight management, nutrition, and answers to their health questions.
Health coaches can help patients of any age. In fact, our health coaches have experience working with parents to improve their children's health through identifying methods of exercise and managing nutrition.
